Medical Form requirements
- Form must be filled out by a doctor under "Clinician's Recommendations" - History Form must be signed and dated by parent/guardian (at the bottom of the first page) - Doctor's stamp must be on form (2nd & 3rd page) - Doctor's registry number aka 6 digit license number on form - Doctor must write date student/patient was seen in the office and sign form - Last page doctor must check "Cleared for all sports without Restriction" Notes on medical: - The Medical form is valid for all sports for one calendar year from date stamped and it is valid until the last day of the expiring month. - If a student is on multiple teams, make a copy of his/her Medical. The original medical form is placed in the most recent sport's Insurance binder. The copy will be placed in previous sport binder for that particular student athlete. Parent Consent for all sports required. - The only accepted medical form is the one issued by the PSAL |
Parent Consent Form requirements
- Form must be written in PEN - Fill out all info at the top (Official Class not mandatory) - Please write at least one phone number on form - All 13 initials must be initialed by parent - Parent must print AND sign name at the bottom - Coach signs bottom of parent consent form and dates it when player has submitted BOTH parent consent form AND medical form correctly - Absolutely NO cross outs or white out on form |

BEFORE THE SEASON
Coaching Requirements (updated 2/26/24) include:
1. Must have a File Number from the NYC DOE
2. AED/CPR by Emergency Skills (every 2 years)
3. First Aid by PSAL (every 3 years) sign up with link here

4. Concussion Training from the CDC or NFHS (every 2 years)
5. Apply for position and submit OP175, resume and letter of intent to Athletic Director & Secretary
Make sure you see PSAL Page for Students to assure students have all necessary requirements finished correctly including:
Parent Consent Form, Medical Form & Printout of Students' Grades on JumpRope
Parent & Student Concussion Info Sheet (given to students prior tryouts)
Transcript (Students must have 5 credit bearing subjects + PE "5 + 1 rule")
To determine your final roster, coach will provide Asst. Principal & Athletic Director with Full Name & OSIS number to print out the Transcripts of the players. Each team may have 1-2 managers that will assist with coach. Managers may practice with team but do not play in games. (Managers must have all paperwork completed)
GRADES
Please print out students grade before the season starts to determine her eligibility
1. Log in to Jump Rope
2. Type in Students' name on the upper right hand corner and click her name.
3. Assure the Mastery button is highlighted in blue and you are able to view students' grades
4. Click on the Silver Gear and wait for the drop down. Then click on Print Screen.
5. Students' grades have been downloaded in to your "Downloads" folder.
6. Open the file and print the document
Attendance (Students must have 90% Attendance Rate) RISA must printed out from the office (Dawn). If student has many absences within a marking period, a parent note/medical must be brought in to excuse the student.
Per Session Coaching Hours for Pre-Season No-Cut Tryouts
(10 Per Session Hours may be paid before Season Starts) this includes the hours allocated.
Per Session Time Sheet
- You may fill this out digitally or write it out at the end of each time period. This must be submitted to the Athletic Director along with the time card and attendance (requires student names & signatures) of participants

A number of questions have come up recently regarding the eligibility of guidance counselors coaching in the PSAL. To help clarify, here are the rules regarding anyone interested in coaching in the PSAL:
- NYC Department of Education Public Schools Athletic League (PSAL) per session coaching positions are open to NYS licensed teachers and supervisors only.
- If a teacher/supervisor is also in possession of a guidance counselor license (and is currently working in that capacity) then the individual would be eligible to apply for a per session coaching position.
