As a student athlete for psal, here's what you need to know/have...
Medical Form requirements updated as of 3/24/2021
The only page students will submit to their Athletic Director or coach is page 3, the “Recommendations for Participation in Physical Education and Sport” page. The “Health History” form (page 1) and the “Physical Examination” form (page 2) must remain on file with the students’ medical provider. Form must be filled out by a doctor Form must be signed and dated by parent/guardian (at the bottom of the first page) Doctor's stamp must be on both Physical Examination forms Doctor's registry number or NPI number is on the form Doctor must write date student/patient was seen in the office and sign form Last page doctor must state "Well Child or Well adolescent" Also acceptable = "cleared for sports" Notes on medical: The Medical form is valid for all sports for one calendar year from date stamped and it is valid until the last day of the expiring month. If a student is on multiple teams, make a copy of his/her Medical. The original medical form is placed in the most recent sport's Insurance binder. The copy will be placed in previous sport binder for that particular student athlete. Parent Consent for all sports required. The only accepted medical form is the one issued by the PSAL |
Parent Consent Form requirements updated as of 3/24/2021
Form must be written in black or blue PEN Fill out all info at the top (Official Class not mandatory) Please write at least one phone number on form All 13 initials must be initialed by parent Parent must print AND sign name at the bottom Coach signs bottom of parent consent form and dates it when player has submitted BOTH parent consent form AND medical form correctly ** 2 Parent Consent form are needed for EACH School Year if applicable: Example, student athlete attends tryouts in June and participates in following September ** |
Hand in the above forms to the Athletic Director or your coach BEFORE TRYOUTS
Please know that if you fail to hand in paperwork on time, you will NOT be invited to attend try outs for the team.
* If you have completed a PSAL Medical form in a previous season, the form is valid for the next season and calendar school year. Please request that your medical form be transferred to your upcoming coach's book
Ex: If your Medical form was completed on 3/1/2021 you may use it for the Fall of 2022.
* If your doctor has given you a physical examination within the calendar year, then the doctor may simply fill out the form (without actually examining you).

Transcript (Students must have 5 credit bearing subjects + PE "5 + 1 rule"
- Please be prepared to provide the following given info to print out your transcript: (Name, OSIS #, & Advisor)
- Assistant Principal (Michael) or Guidance Counselor will print out your transcript
Attendance (Students must have 90% Attendance Rate) RISA must printed out from the office (Dawn). If student has many absences within a marking period, a parent note/medical must be brought in to excuse the student.
